Background: RE is a benign laryngeal disease involving diffuse polypoid degeneration of the vocal folds, most commonly preceded by chronic tobacco use. Morphologic changes of the vocal folds are the result of edema, vascular congestion, and venous stasis within the superficial layer of the lamina propria, known as Reinke’s space.

Reinke’s edema occurs when the layer under the surface lining of the vocal fold - called Reinke’s space - fills with fluid due to long-standing vocal trauma, most often from smoking. In the most severe cases, the entire membranous portion of the vocal folds becomes filled with thick, gelatinous fluid, and the vocal folds look like enlarged fluid-filled balloons/elephant ears.

Reinke’s edema usually develops over years, and so a patient (and even family and friends) may not notice the gradual progression over time. Surgical Treatment of REINKE’S EDEMA While cessation of smoking and anti-reflux treatment may be helpful in reducing progression over time, typically, surgical reduction of the inappropriately massive vocal cords is required to treat Reinke’s edema. Symptoms: Reinke’s Edema causes a lowering in the pitch of the voice. This change in voice is often more noticeable in women. Occasionally, the swelling can be significant and can obstruct the larynx, resulting in difficulty breathing. Reinke edema is characterized by increased fibrin and vasculature, as well as accumulation of gelatinous fluid in Reinke's space (figure 2). The vocal folds may appear swollen, floppy, or polypoid; hence, alternative names such as polypoid corditis, polypoid degeneration, and edematous hypertrophy have been used to describe this process.
